Express Visit
15 minutes — $60
A focused visit for one straightforward concern.
Express Visits may be appropriate for a minor illness, simple injury, rash, brief medication question, or focused follow-up.
New and established patients may schedule an Express Visit. Additional concerns or more complex symptoms may require a longer appointment.
Focused Visit
30 minutes — $100
Designed for one or two health concerns, routine follow-up care, medication management, a focused laboratory review, or ongoing management of a stable condition.
This visit provides time to discuss your progress, answer questions, and make focused adjustments to your care plan.
Extended Visit
45 minutes — $150
Appropriate when you need more time to address several concerns, review laboratory findings, discuss lifestyle factors, or make meaningful changes to an established treatment plan.
Extended Visits may include primary care, preventive care, integrative or functional medicine, and wellness counseling.
Comprehensive Visit
60 minutes — $200
Our standard comprehensive appointment and the most common visit length at Driftless Health.
This visit provides time to understand your concerns, review your health history, address multiple needs, and develop a thoughtful care plan.
It may include primary care, prevention, chronic-condition management, integrative or functional medicine, nutrition, lifestyle counseling, medication management, or detailed laboratory review.
Comprehensive Deep Dive
90 minutes — $275
Designed for complex or long-standing concerns that require a more detailed evaluation.
This appointment may include review of multiple body systems, an in-depth health timeline, extensive laboratory findings, nutrition and lifestyle factors, environmental influences, and development of an individualized integrative or functional medicine plan.
Extended Deep Dive
120 minutes — $350
Our most comprehensive appointment for patients with complex health histories, multiple ongoing concerns, extensive outside records, or a need for detailed functional and integrative care planning.
This visit allows time to explore the larger picture of your health and create a structured, personalized plan with clear priorities and next steps.
Sports or Participation Physical
30 minutes — $50
A focused examination for participation in school, recreational activities, camps, employment programs, or organized sports.
The visit includes review of the required history, a physical examination, and completion of the standard participation form when appropriate.
Additional medical concerns, testing, treatment, or extensive forms may require a separate visit.
DOT Medical Examination
60 minutes — $150
A Department of Transportation medical examination completed in accordance with Federal Motor Carrier Safety Administration requirements.
The fee includes the examination and completion of required DOT documentation.
Additional testing, outside records, specialist evaluations, or services needed to determine certification are not included. Completion of the examination does not guarantee certification.
New Patient Visits
A one-time $50 new-patient fee is added to the first clinical appointment at Driftless Health.
This fee supports review of intake information, medication and health-history reconciliation, creation of the patient record, and preparation for the first visit.
New patients may choose the appointment length that best matches their needs. Patients establishing ongoing primary care or beginning a comprehensive integrative or functional medicine evaluation will commonly select a 60-minute or longer appointment.
New patients may also schedule shorter visits for appropriate focused concerns.
The new-patient fee does not apply to stand-alone sports or participation physicals or DOT medical examinations.
Appointment Deposits
A deposit of $35 is required when scheduling.
The deposit is applied in full toward the completed visit.
Appointments must be canceled or rescheduled at least one business day before the scheduled appointment time.
Deposits are forfeited for missed appointments or late cancellations, and a new deposit may be required before rescheduling.
What Is Included in the visit price?
Visit fees cover the professional evaluation, counseling, care planning, and routine documentation provided during the scheduled appointment.

Point-of-care testing
Procedures
Medications or injections
Medical supplies
Specialty functional testing
Imaging
Outside laboratory or specialist services
Certain administrative or extensive forms
Whenever reasonably possible, known additional costs will be discussed before the service is provided.
Telehealth Appointments
Telehealth and in-office clinical visits use the same time-based pricing.
Telehealth may be available for eligible primary care follow-ups, medication discussions, laboratory reviews, wellness visits, integrative or functional medicine appointments, and other concerns that can be safely addressed remotely.
An in-person visit may be recommended when an examination, testing, or hands-on evaluation is needed.
Direct Pay Care
Driftless Health does not bill or accept health insurance, Medicare, or Medicaid.
Patients pay Driftless Health directly for appointments and additional services.
Patients may independently ask their insurance plan whether a service is eligible for out-of-network reimbursement, but Driftless Health cannot guarantee coverage or reimbursement.
