Meet Your Provider

Jacob Schultz is a board-certified Family Nurse Practitioner and the founder of Driftless Health. His clinical background includes nearly two decades in emergency and critical care, including rural emergency care and critical care transport. Through years of caring for patients during serious illness, injury, and moments of crisis, Jacob has seen firsthand how important it is for healthcare to be timely, thoughtful, practical, and deeply personal. He has also seen how often care begins only after disease has already progressed, when long-term health also depends on prevention, early support, and attention to the broader factors that shape well-being.

Driftless Health was created from the belief that patients deserve more time, more clarity, and a care experience built around the whole person. Jacob’s approach combines conventional primary care principles with wellness-focused prevention, lifestyle support, health coaching, appropriate evaluation, and evidence-informed integrative care. He works with patients to better understand their health concerns, identify realistic next steps, and create shared care plans that reflect their goals, values, and long-term well-being.

Jacob completed his Family Nurse Practitioner education through the University of North Dakota and is currently completing a PhD focused on rural healthcare. His education and clinical experience reflect a strong commitment to improving care access, prevention, and long-term health in rural communities.

Rooted locally in Vernon County for more than 10 years, Jacob is committed to serving Viroqua and surrounding Driftless communities with care that feels personal, grounded, and connected to real life.
